A mother died within three hours of giving birth after an epidural anaesthetic was mistakenly fed into her arm instead of her back, an inquest has heard.

Mayra Cabrera, 30, originally from the Philippines, suffered a heart attack after being given the drug during the birth of her healthy 8lb baby Zachary. Mrs Cabrera died on May 11, 2004, at the Great Western Hospital in Swindon, Wilts, where she had worked as a theatre nurse since moving to Britain in 2002.

A new father has died after catching the common children’s virus chickenpox at the age of 37.

University technician Geraint Hughes had been ill for a week after catching the virus, but doctors warned his wife that chickenpox can be serious for adults - but is rarely fatal.

However Geraint, who had an 18-month-old baby, died after developing complications.
