I was one of the 12 whose kids did fall out due to the straps pulling through. Fortunately, I caught my baby before he would have hit the ceramic tile floor. I sent an email to RC2, and they didn't bother to respond. So I filed a complaint with CPSC.

The straps are held on in the back by a little tab, half an inch on either side, made out of nylon strapping. Worked fine at first, but after a few months, it completely collapses and the straps pull right through.
